Adding Fields on the page.

a. Field Tab:

 

1. Enter Field Name Field Name will show up in the Tree Menu under a page and not on the survey/form. and check if it’s a Required Becomes a mandatory question on the form. Validation should be turned on in form settings so this field is validated every time user transitions between pages or submits the form. field.

2. Type in Label This is the label or question text. You can also insert images by using the HTML editor. information

3. Select Field Type Sets the type of field according to the selection. The field can be Option Button, Text Field, Check Box, Drop Down, Label Field Label Field does not require response from the survey respondent. A label Field can be created to place some text or image within the form. For example, if you wish to display a picture of a product, create 'Label' field, and use HTML editor to insert an image. or Hidden Field (Use a hidden field to define a value that does not show up on the survey. For example, you may want to 'code' a survey to a particular department or company. By including a hidden field, set with this value, you can use the code in a report.).  

4. Select Type Options Provides you with more options to add with the selected field type. Like, with option button & check boxes, you have the option to have the last label as 'other' or also have a text field with the other label so that the user can also specify their other option. With the Text Field you get the option to specify the format from the drop down list if it matches the requirement or you can specify the max & min lengths of the fields and check the Multi-line checkbox if you require to give the option of selecting multiple responses in the Drop down list. , if required.

 

 

b. Response Tab:
In case of option button, check box or drop down you'll have to enter response label and values.

1. Choose Number of responses Number of possible responses you want to provide with this field. (maximum: 200)

2. Choose default response number (Choose either the number of the default response or no default response)

3. Specify Value Values can be used in different ways in Forms.

    Value shows up when you run the reports.

 Value Field can be used to alias your Label.  For example, Label can be Human Resources while Value can be HR.
Value Field can be used to give a certain value to a choice.  For example, 0 for Male and 1 for Female.

 

Certain characters cannot be put in the value field such as apostrophe ' and '&' sign.             

 

4. Response Label It is the label of response. Use Edit to edit/customize label font, color, size etc. If you want the label of the response to be same as the value, leave the label field blank. fields - Enter the label of the response that will appear on the survey. If response label is same as the value, there is no need to specify the label because if the label is not specified,  the value will automatically appear on the survey.

* Note - Leave blank to make Label be the same as Value.

5. Use Edit to edit/customize label font, color, size etc.

 

     

     The responses whose labels are not filled in will

           automatically take the value as it's label.

 

 c. Options Tab:

1. Encrypt this response The response entered is saved encrypted and can only be decrypted if you export the data & use the decrypter to decrypt it. - check if encrypted response is desired.

2. Select Choices Across Sets the number of response choices to appear horizontally in one line. * Note that this feature can only be used with option button & check box fields. - Choose number of responses to display horizontally.

3. Field List Numbers: 1, 2, 3
Alpha: A, B, C
Bullets:  ·
- Select if you want your field to be listed in numbers, alpha A,B,C...., or bullets.

4. Select Choice List if you want to list the response options of the field in numbers, alpha or bullets.

5. Label Width - Enter width of label/question. * Please note that these are in pixels.

6. Align Label - Choose text alignment for label/question.

7. Field Width - Enter area width of answer/response. * Please note that these are in pixels.

8. Align Field - Choose text alignment for response.

9. Align Label and Field - Check to put the question and the responses in one line.

10. Align This Field With the Last - Check to put the current field on the same line as the previous field.

11. Line Space Before Field - Enter number for the number of line spaces you would like to have above this field. By default, before fields value is 0.

12. Line Space After Field - Enter number for the number of line spaces you would like to have after this field. By default, before fields value is 1.

 

d. Matrix Tab:

1. If you are making a Matrix. You can select the number of matrix fields from the drop down list provided.

2. Unique Responses - Select, if you want the responses to be rated uniquely.  In order to use unique responses effectively,  make sure that the number of your matrix fields is equal to the number of your rating (response) fields.  

3. Name filed: You can enter the matrix attributes here.

4. Label fields - If left blank, It will default the value of Name field.  

* The only difference between Name and Label Fields is  that you can use HTML editor on the text enter in Label Field.

5. Edit Button - For advance formatting using HTML Editor.

 

                                                                            

e. Pipes Tab:

A pipe takes a value from a previous field and inserts it into another field label. The value must be from a field on a previous page.  Ioxphere allows three pipes per field.

*NOTE:

1. Piping is only possible on multiple-page surveys/forms.

2. Piping is not possible on first page of the survey.

3. It is not possible to pipe to or from the same page.

4. It is not possible to pipe from fields of type "Label" because these fields do not contain responses.

a. Replace this word: Choose unique word to replace (i.e. name, or insert-name-here)

b. With response from: Choose a field name from the drop down menu.

f. Fonts Tab:

1. Field Font Type - Choose a font for the field.

2. Choice Font Type - Choose a font for the choice/response.

 

You should only use this option, if you want to override the Fonts specified in Forms settings for this question.

* You can leave these options blank, if you want to use the same fonts as specified on Form Settings.

g. Colors Tab:

1. Field Background Color - Choose a color for the field background by clicking on Edit, and choosing a color.

2. Field Foreground Color - Choose a color for the field text by clicking on Edit, and choosing a color.

3. Choice Background Color  - Choose a color for the choice/answer background by clicking on Edit, and choosing a color.

4. Choice Foreground Color  - Choose a color for the question choice/answer text by clicking on Edit, and choosing a color.

 

You should only use this option, if you want to override the colors specified in Forms settings for this question.

* You can leave these options blank, if you want to use the same color as specified on Form Settings.

 

h. Help Tab:

1. Field Help - Design custom help screens for use with particular fields (questions).

2. Edit -  Use this to edit/customize page text, fonts, color, etc.

 

* To have a look at the form, you can Preview the form.
