Creating a New Form

OR

a. Settings Tab:  (Here you will add the Global Setting of the form you are creating.)

 

1. Enter the Form Name This is the name of the form for your own reference (the survey respondent does not see this).  Form Name will appear on the tree menu as soon as you save the form. and the Form Title Title of the form/survey that is displayed on top of each page of the form..

2. Choose a Form Category.

3. On Submit go to is the 'Web destination' your form users will be directed to when they click on the Submit button.  There are four choices available:

4. If you want to send results to an email address, enter that email in the On Submit email to field.

5. Select who can edit the form from the drop down list of This form can be edited by This option gives survey management rights to other departments created by you. To use this option you must have departments created under your account else leave this value as the default value:.

6. Block Multiple IP: Every Computer on the internet is recognized by its IP (Internet Protocol) address. if you check this option, users from the same IP address(computer) can take the survey only once.

7. Check Allow to Finish Later Adds a button on the form/survey that allows the user to fill the form in portions if desired. When the user comes back to the form he/she can continue further from where they left rather than starting from the beginning. Finish Later button stores responses for 30 days. checkbox, if you want the Finish Later button to appear on the form (this allows a form user to save their responses and finish form at a later time).

8. Check Show Receipt The Receipt is used to display a 'pop-up thank you' and gives the user a certificate code. You can use this unique code for rebates and coupons. check box if you want generate a receipt for the user on submission of form.

9. Select Form User Type (Anyone = public access). If you have Departments with in your account, you can give access to certain departments

10. Choose Validate On:

* Keep in mind, if you plan to make certain fields "Required" in your form, you should select a validation option here otherwise the required fields won't be validated.

11. If you want to include your company logo on the survey, click the Edit button next to Logo, then in the Select Image Dialog Box, click on New and browse for your logo file.  If you no longer want to keep the logo on the form, click Delete to remove it.

12. Repeat step 11 if you want a Background Image on your survey.

 

    

  

b. Security Tab:

1. Select Protocol, http:// for non- secured pages, or https:// to use Secure Sockets Layer Security.

2. Type in the Encryption Key if you plan to encrypt any of the text fields. *You'll need this encryption key to decrypt the encrypted fields.

 

 

c. Style Tab:

1. Form Style - Select whether you want Standard Form This is the basic form format where the next & previous buttons are used to move between the pages., Tab Form Each Form page shows up as a Tab. Submit Button is on the last tab/page. or Double Tabbed Form The form has the same tabs on both the top and bottom of the page.

2. Number of columns - Choose 1 or 2 columns for the arrangement of questions/statements.

3. Choices Across Column - Select the number of choices the user will have to select from (1 through 8).

4. Field List Style – Select if you want fields(questions) to be displayed as numbers, alpha or bullet.

5. Choice List Style - Select if you want the choices to be displayed in numbers, alpha  bullet or none.

6. Check Show Page Numbers checkbox, if page numbers should be shown on form.

 

 

d. Buttons Tab:

1. You can arrange the “Submit”, “Next”, “Previous”, “Reset” and “Finish Later” buttons horizontally on your page

2. The text on each of these buttons (“Caption”) can also be changed

 

 

e. Fonts Tab:

1. Select the Page Font Type.

2. Select the Field Font Type.

3. Select the Choice Font Type.

* You do not have to choose font, you may simply use the default font.

 

f. Colors Tab:

1. Page Background - Choose a color for the form (page) background by clicking on Edit, and choosing a color.

2. Page Foreground - Choose a color for the form (page) text by clicking on Edit, and choosing a color.

3. Field Background - Choose a color for the field background by clicking on Edit, and choosing a color.

4. Field Foreground - Choose a color for the field text by clicking on Edit, and choosing a color.

5. Choice Background - Choose a color for the choice/response background by clicking on Edit, and choosing a color.

6. Choice Foreground - Choose a color for the choice/response text by clicking on Edit, and choosing a color.

 

g. Tab Colors Tab:

1. Background - Choose a color for the tab form page background by clicking on Edit and choosing the color.

2. Foreground - Choose a color for the tab form page foreground type by clicking on Edit, and choosing a color.

3. Selected Background - Choose a color for the selected Tab background type by clicking on Edit, and choosing a color.

4. Selected Foreground - Choose a color for the selected Tab foreground type by clicking on Edit, and choosing a color.

5. Unselected Background - Choose a color for the unselected Tab background by clicking on Edit, and choosing a color.

6. Unselected Foreground - Choose a color for the unselected Tab foreground type by clicking on Edit,  and choosing a color.

* Note: Selected and unselected background and foreground colors are just for the tabs on the form not the page of the tab form.   

 

h. Receipt Tab:

1. Background - Choose a color for the background of receipt by clicking on Edit, and choosing a color.

2. Foreground - Choose a color for the foreground type of receipt by clicking on Edit, and choosing a color.

3. Thank you text - Type in a thank you or congratulations message, etc.

4. About text - Use this field to designate the header for your certificate.

5. Certificate Text - Use this field to assign your user a unique certificate number to use for coupons or rebates.

6. Click on Show button to preview the receipt layout.

 

 

 

 

 * After defining global setting of the form, you can Add Pages and Fields to your form.
